Thornwood believes in providing quality daycare for your pet. We are dedicated to providing your pet with a fun and positive experience. We go to great lengths to ensure the safety of your pet, however, along with the benefits of socialization and exercise, there is risk anytime dogs participate in group play.
This agreement is a release of liability and waiver of legal rights and deprives you of the right to file a lawsuit against Thornwood Veterinary Clinic for injuries or disease your pet may sustain or contract while participating in daycare. It is also an agreement that you are financially responsible for your own dogs injuries or disease treatment should they require medical care. Do not sign this agreement unless you have read and understand it in its entirety. Seek the advice of legal counsel if you are unsure of its effect.

Policies and Conditions for Admission to Dog Daycare: 1. I understand and accept that my dog will be handled and cared for by Thornwood's staff without liability for loss or injury. 2. I understand and accept that sending my dog to daycare increases risk of communicable diseases such as kennel cough and others and that I am financially responsible for treatment of my pet should it require medical care. 3. I understand and accept that allowing my dog to participate in daycare increases the risk of injury from altercations with other dogs as well as from simple physical exertion. This is a no blame environment, in the event of an altercation with another dog or any other injury caused from physical activity, I am financially responsible for my own pets injuries and any needed medical care. 4. Thornwood is committed to the highest quality care for your dog and all the dogs participating in daycare activities. Your dogs temperament and group interaction will be under constant review. We reserve the right to refuse a pet into the program and/or expel a pet from the program at any time, for any reason. 5. It is understood that Thornwood reserves the right to use pictures, video and/or other media records of in the promotion, advertising, sale, publicizing and exploration of Thornwood Veterinary Clinic, Kennel and Daycare operations. Once again, this agreement is a release of liability and waiver of legal rights and deprives you of the right to file a lawsuit against Thornwood Veterinary Clinic for injuries or disease your pet may sustain or contract while participating in daycare.
